• Minimum of two Professional References (Clinical Nurse Manager and Most Recent Employer)
• Health Declaration Form and Proof of Immunisation Status for Hepatitis B, TB, Rubella.
• Work Permit/Visa details if applicable
• Professional Certificates of Qualifications (Including all Mandatory Courses)
• Copy of registration with An Bord Altranais and Statement of Entry/Registration Details
• Proof of Professional Indemnity Insurance (e.g. Copy of INO or SIPTU Membership Card)
• 2 Signed Passport Sized Photographs in order to complete an ID Badge
• Passport/and or Birth Certificate
• Proof of Address x 2 (. e.g. Driving Licence/Bill/Bank statement)
• Bank Details
• Garda Clearance if you have one or clearance from another country
